Coquille Indian Tribe Community Health Center
Mission Statement To promote the wellness of the community through medical outpatient, limited optometry, health promotion/disease prevention, and diabetes management and education. This community is comprised of all Coquille Tribal family members, Native Americans, and the general public. Coordination of Social Services, Contract Health Services and Community Health are available to Coquille Tribal families. History The Community Health Center was developed and administered based on a Health Plan and Community Health Risk Survey developed by the Tribe. The Health Center first opened in 1995.The Community Center was built in 1998 and opened in 1999. Accreditation The Coquille Indian Tribe Community Health Center (CITCHC) is recognized by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC). Achieving AAAHC accreditation demonstrates a high level of commitment and effort and demonstrates the Coquille Indian Tribe Community Health Center’s dedication to providing quality health care services. The Community Health Center initially received accreditation in 2001 and has subsequently passed national reviews in 2004, 2007 and 2010.
